PRIVACY POLICY WITH SMS TERMS & CONDITIONS AND STANDARD MESSAGING DISCLOSURE

Brookhaven Counseling Group uses the number 601-822-2286 to receive and respond to text messages with clients concerning their appointments and/or paperwork.

CONSENT TO RECEIVE/SEND TEXT MESSAGES USING THE iPLUM TEXT MESSAGING (SMS) SERVICE:

iPlum IS THE TECHNOLOGY SERVICE WE WILL USE TO CONDUCT SMS MESSAGES. It is simple to use and there are no passwords required to log in.

  1. I understand that my health care provider wishes me to engage in texting for appointment reminders/communication.

  2. I understand that a text message with my provider and, or an employee of Brookhaven Counseling Group has potential benefits including easier access to care and the convenience of making/confirming/cancelling appointments.

  3. I understand there are potential risks to this technology, including interruptions, unauthorized access, and technical difficulties. I understand that my health care provider and or employees can discontinue text messaging if I do not feel the service is necessary or adequate.

  4. I have had a direct conversation with my provider and, or employee, during which I had the opportunity to ask questions in regard to this procedure. My questions have been answered and the risks, benefits and any practical alternatives have been discussed with me in a language in which I understand.

  5. iPlum SMS is NOT an Emergency Service and in the event of an emergency, I will use a phone to call 911.

  6. Though my provider and I may be in direct texting contact through the iPlum Text Messaging (SMS) service, the provider does not provide any medical or healthcare services or advice including, but not limited to, emergency or urgent medical services.

  7. The iPlum Text Messaging (SMS) service is not responsible for the delivery of any healthcare, medical advice or care.

  8. To maintain confidentiality, I will not share my text messages with anyone unauthorized to read the messages concerning appointments.

  9. I understand, that the iPlum Text Messaging (SMS) service is HIPPA compliant and none of my information will be shared with anyone, for any purpose.

  10. I understand, that Brookhaven Counseling Group providers or employees do not collect identifiable information through the iPlum Text Messaging (SMS) service.

  11. Personal information stored in iPlum SMS messages will NOT be used for any purpose other than the direct SMS client communications.

  12. I understand, phone numbers collected with the iPlum Text Messaging (SMS) service will not be shared with third parties or affiliates for marketing purposes under any circumstances.

  13. By giving consent to SMS communication, information obtained as part of the SMS consent process will NOT be shared with third parties.

  14. Types of SMS Communications: If you have consented to receive text messages from Brookhaven Counseling Group, you may receive text messages related to the client’s appt-see example below:

  • Appt Reminder: You have an appointment tomorrow with ______ at Brookhaven Counseling Services @ 10 a.m. Please let us know if you will be at your appointment or if you need to reschedule.

  • Client Reminders: You must complete your paperwork before your scheduled appt on Dec. 20, 2025 @ 10 a.m. If you have questions, please give us a call at our office, 601-822-2286.

    • You agree to receive informational messages (appointment reminders, account notifications, etc.) from Brookhaven Counseling Group. Message frequency varies. Message and data rates may apply. For help, reply HELP or email us at contact@brookhavencounselinggroup.org. You can opt out at any time by replying STOP.

    • As a current or prospective customer, you understand that you can text us STOP at any time to opt out of receiving SMS text messages from us. You can text us HELP at any time to receive help.

    • You understand that the messaging frequency may vary. Messaging & data rates may apply.

    • Your mobile information will not be shared with any third parties/affiliates for marketing/promotional purposes. All policies are followed as per CTIA guidelines 5.2.1. At any time if you want your information to be removed, you can contact us via our email address or regular mail.

Clients are given this policy to sign after verbal permission is given to send you an email with paperwork and/or a text message concerning upcoming appointments. By opting into SMS from a web form or other medium, they are agreeing to receive SMS messages from Brookhaven Counseling Group LLC. This includes SMS messages for appointment scheduling, appointment reminders, post-visit instructions, lab notifications, and billing notifications. Message frequency varies. Message and data rates may apply. By signing this form, clients certify: They have read or had this form read and/had this form explained to them; They fully understand its contents including the risks and benefits of text messaging; They have been given ample opportunity to ask questions and that any questions have been answered to their satisfaction; and agree they have read, understood, and agree with the items in this document.
